Catechism Registration – Parent/Guardian Consent & Agreement

1. Supervision & Responsibility

I/We understand and agree that it is my/our responsibility to supervise my/our child before and after scheduled catechism classes, Holy Qurbana, and any parish or church-related activities. I/We acknowledge that the Mission, its priests, catechism teachers, and volunteers are not responsible for the supervision or safety of children outside officially scheduled program times.

If my/our child is under 18 years of age, I/We confirm that either I/We or a responsible adult guardian will accompany and collect the child when leaving classrooms or designated parish areas.

2. Timely Pick-Up & Sign-Out Requirement

I/We agree to personally collect and sign out my/our child after catechism classes if not attending Holy Qurbana, ensure timely pick-up at the conclusion of classes or Mass, and inform the catechism teacher or Principal in the event of an unexpected delay.

I/We understand that children are not permitted to go to the carpark or pick-up zones without direct parent/guardian supervision.

3. Faith Formation Consent

I/We consent to my/our child attending catechism classes and receiving instruction in the Catholic faith, including prayers, traditions, and values, as part of the Catechetical Program of St Mary’s Syro-Malabar Mission. I/We agree to support the catechism teachers and volunteers in the faith formation of my/our child and endeavour to participate regularly in Holy Qurbana.

4. Parish Registration & Communication

I/We agree to register our family in the Mission’s official parish records and consent to being contacted by the parish priest or authorised church representatives via phone, email, or messaging platforms (including WhatsApp) for matters related to parish life, catechism coordination, faith formation, and pastoral communication.

5. Medical Emergency Consent

In the unlikely event of a medical emergency during catechism classes or parish activities, I/We consent to being contacted and authorise catechism teachers or volunteers to arrange basic first aid and/or emergency medical care, including transport to a hospital if required. I/We understand that any costs arising from such medical treatment are my/our responsibility.

6. Photography, Video & Media

I/We acknowledge that photographs or video recordings may be taken during catechism classes and parish activities for parish publications, educational materials, the parish website, and official social media platforms. The Mission commits to ensuring that all media is captured and used respectfully, safely, and appropriately, with due regard for the privacy and dignity of children.

7. Accuracy of Information & Agreement

I/We confirm that all information provided in this registration form is accurate and complete and agree to comply with all policies, guidelines, and instructions issued by St Mary’s Syro-Malabar Mission and the Catechetical Department.
